Please use this form to provide notification to the school of your child’s early dismissal. Please note that this notification will be reviewed in accordance with LMSD Policy and Administrative Regulation #207.

The achievement of your child at school, in which no doubt you are sincerely interested, depends greatly on regularity and punctuality of their attendance. This notification will be reviewed in accordance with LMSD Policy and Administrative Regulation #207.

Parents/Guardians of students who are absent will receive a verification phone call and email, even if the absence has been reported via our LMSD Safe Arrival system. Phoning parents and guardians of students who are not in class is an important measure undertaken by our school district to ensure the safety of all of our students.

If you have a doctor’s note that would excuse this absence, please send the original note to the school office upon the child’s return.
