• (2) PROOFS OF RESIDENCY: Documentation must be current and in the Parent/Guardian’s name
  • Property Deed, Mortgage Statement or Real Estate Tax Bill and a current Utility Bill
  • Signed Lease Contract and a current Utility Bill or Renter’s Insurance Policy
• IMMUNIZATION RECORDS: Complete immunization history is required for all students in grades Kindergarten through 12^th.

• PHYSICAL EXAM: A recent physical exam, completed within the last calendar year, is required for students entering grades Kindergarten, 1^st, 6^th and 11^th.

• DENTAL EXAM: A recent dental exam, completed within the last calendar year, is required for all students entering grades Kindergarten, 1^st, 3^rd and 7^th.
