Request for Transcripts*

Please follow the guidelines below to request a copy of your transcript.

If you graduated in the past five years, contact your high school directly:

• Lower Merion High School: Katrisha Hairston, Registrar - hairstk@lmsd.org
• Harriton High School: Kate Wosczyna, Registrar - wosczym@lmsd.org

If you graduated more than five years ago, the student requesting the transcript must print out this form, fill it out and sign it. Be sure to include date of birth, graduation year and student name at the time of graduation.

Returned the signed form by:
