Pathway Change Request Form

At this time, class assignments for the 2020-2021 school year have been finalized. This includes assignments for students who are following the pathway to return to in-person instruction, LMSD@Home and LMVA.

All new requests to change a student from one pathway to another must be reviewed by the LMSD Administrative Team (K-8) and the High School Administration (9-12) to determine placement.

In order to make such a request, families must complete this LMSD Pathway Change Request Form.
