Lower Merion School District requires that a child entering Kindergarten must have reached his/her fifth birthday on or before September 1.
Registration procedures are not complicated, but must be followed closely. Before your child can be scheduled for classes, the information requested in the following paragraphs must be provided. Be sure that you are registering your child(ren) in the proper school(s). Personnel in each school will help you, but if you need additional assistance, call (610) 645-1800.
- Evidence of Immunization and completion of the PA Certificate of Immunization: The Pennsylvania Department of Health regulations require the exclusion from Pennsylvania Schools of any student(s) who do not meet the following minimum immunization Requirements:
- Four or more properly spaced doses of Diphtheria and Tetanus (DTP, DTaP, Td DT vaccine.)- one dose administered on or after the fourth birthday.
- Three doses of Polio vaccine.
- Two properly spaced doses oflive Measles vaccine. The first dose must be administered after 12 months of age (or blood test showing immunity). A second dose of measles vaccine, preferably given as MMR (measles, mumps and rubella) is required.
- One does of Rubella (German measles) vaccine after 12 months of age (or blood test showing immunity).
- One dose of Mumps vaccine after 12 months of age (or blood test showing immunity).
- Three doses of Hepatitis B vaccine, the first two doses given one month apart and the third dose six months after the first dose.
- Evidence of Varicella (Chicken pox) immunity: Date of Varicella disease or vaccine-one dose of Varicella vaccine administered at 12 months of age through 12 years of
age; or two doses of Varicella vaccine administered at age 13 years or after.
Children who have a medical condition that would contraindicate certain immunizations may be exempted from immunization requirements. This exemption requires a written statement from the physician documenting the reason for exemption.
Parents/Guardians who have a religious objection to immunization may indicate this on the PA Certificate of Immunization along with their signature. Students with immunization exemptions may be excluded from school at the direction of the Department of Health should an outbreak of the disease for which they are not immunized occur.
- Tuberculin Testing: Routine tuberculin testing is not required for enrollment to school. Tuberculin testing may be required if a student is in a high risk group (as defined by the Centers of Disease Control) for contracting tuberculosis. The School Nurse will contact you if tuberculin testing is required.
- Proof of Residency: LMSD must be certain that the parent/guardian’s residence, with whom the child lives, is in Lower Merion Township or the Borough of Narberth. Any of the following documents will verify your LMSD residency:
- A signed lease.
- A copy of the Deed, Real Estate Tax Bill or Sewer rental Bill
- Utility bill (gas, electric, oil, water, phone or cable)
- Evidence of place and date of birth: (one of the following must be presented)
- Birth Certificate, Baptismal Certificate or Passport
- Completion of Physical Examination Form (exam by private physician) on entrance to school (Kg or grade 1), in grade 6 and grade 11. The exam must be done within the school year for which it is required. If your child does not have a private physician or does not have medical insurance, please notify the school nurse or school counselor so that they may assist you.
- Completion of Family Dentist Report: The form must be completed and signed by the family dentist for students upon entrance to school (Kg or grade 1), grades 3 and 7. It is strongly recommended that your family dentist perform the exam as he/she is the most familiar with your child’s dental needs and will be able to provide follow up treatments, cleanings etc. A written request must be submitted to have your child receive an abbreviated visual exam by the School District consulting dentist.
- Completion of Pupil Registration Form, LM-3.
- At the time of registration, please sign your name to the form requesting school records from the school your child previously attended.
- If you are the custodial parent, or guardian or foster parent, please present documentation of custody, court or placing agency verification.
If you believe your school-aged child seems to be developing differently than other children, and you desire information concerning possible Special Education services, please call 610 645-1800 (also indicate on the Registration Form that your child may need Special Education).
